Driving evaluation and vehicle adaptation progra

CISSS de la Montérégie-Ouest

The Driving evaluation and vehicle adaptation program is intended for drivers and passengers with a motor impairment who want to get around easier in the community and improve their functional autonomy. There are two components to the program:

Driving evaluation 

For drivers or learners who want or require a professional assessment of their functional capacity to drive a class 5 passenger vehicle.

Vehicle adaptation

For drivers, learners or passengers who want or require a professional assessment of their needs in terms of adapted driving, access to the vehicle, and/or loading a mobility aid.

How to make a service request for...

A driving evaluation, if you are a driver or a learner:

  • Fill out the service request form, including a description of the need justifying the referral to the driving evaluation program;
  • Provide a recent medical report containing a detailed list of the medical conditions (form M-28 is accepted as a medical report and will be required to initiate services).

Vehicle adaptation, if you have a driver’s license:

  • Fill out the service request form, including a description of the need justifying the referral to the driving evaluation program;
  • Provide a recent medical report containing a detailed list of the medical conditions (form M-28 is accepted as a medical report and will be required to initiate services).

Vehicle adaptation, if you do not have a driver’s license:

  • Fill out the service request form, including a description of the need justifying the referral to the driving evaluation program.
