What is post-hospitalization follow-up? 

Were you recently discharged from the hospital? Did you know there is a team available to answer your questions and address your concerns about your health, and also to coordinate the follow-up you need to help you recover and avoid another hospital stay or trip to the emergency department? 

It is a telephone helpline staffed by a hospital nurse. 

  • The line is open Monday to Friday, from 9 a.m. to 5 p.m. 
  • This service is available for 30 days after you are discharged. 
  • In an emergency, call 911.

Why call us? 

  • You are not feeling well, your condition is worsening, or you have new symptoms that you are worried about. 
  • You want to know what to do about a worrisome situation (e.g., your dressing comes off accidentally). 
  • You need a referral to an appropriate resource in the healthcare network.
