You must have your health insurance card and your valid requisition form. Any requisition form that is not identified, not signed, incorrectly filled out, or not from Québec will be refused.

If your requisition is urgent and no time slot is available, you can call 1-833-699-2414 from 8 a.m. to 4 p.m., Monday to Friday, and we will assess your situation.

Note that some tests are not available at the Candiac local point of service. We recommend checking your requisition carefully before making an appoinment. If one of the following tests is mentioned on your requisition, please make an appointment at your CLSC.

• Glucose challenge test with 50 g and 75 g glucose
• 24-hour urine collection
• Urine cytology
• Chlamydia and gonorrhea urine test
• Stool culture
• Stool parasite test
• Cortisol saliva test
• Whooping cough test
• H. pylori test

Seniors aged 80 and over as well as immunosuppressed people aged 18 and over can get the shingles vaccine for free. Find out more.

Vaccination of PPD immunodepressed people (transplant, vaccination schedule prior to immunosuppressive therapy) is offered exclusively at CLSCs.

Vaccination for babies aged 0-2 is offered exclusively at CLSCs.
