Description

To obtain a summary of your medical record or a specific document from your medical record, you must send a duly signed written request to our Medical Records department by fax or by mail to the following address:

The request must contain the following information:

  • your family name and first name;
  • your date of birth;
  • your medical record number (if available);
  • your complete address;
  • your telephone number;
  • the facility where you received care;
  • the nature of your request (clearly indicate which documents you require);
  • the date by which you need the information.

Then, sign the request and indicate the date of the signature (recent). In compliance with the Act respecting access to information, a fee is payable in advance for certain services.

Take the elevator to the basement and turn right. The Medical Records department is at the end of the corridor.

Description

To make your stay easier, we recommend you read these tips and instructions before you arrive at the hospital.

The day before your surgery

Preparing yourself, body and mind

  • Do the preparations requested by your surgeon, where applicable.
  • Since you need to be fasting, follow the instructions given during the preadmission appointment (recommendations about medications, eating and drinking).
  • Maintain a positive attitude and talk about your concerns with a family member or trusted friend. Get a good night’s sleep.
  • Don’t do any strenuous activity. Instead, rest, relax and take naps.
  • Take a shower or bath to clean your skin and prevent infections.
  • Avoid coffee and alcohol.
  • Establish your recovery plan, including the help you’ll need with your personal hygiene, meals and housework.

Support person

Make sure a responsible person is with you on the day of the surgery and stays with you the first night.

What to pack in your suitcase

  • Your health insurance card
  • Your hospital card
  • Your usual medications, in their original containers, including over-the-counter medications and natural products
  • A robe and non-skid slippers
  • Clothing that’s easy to put on and appropriate for your type of surgery
  • Cases for your dentures, eyeglasses and contact lenses, where applicable

Do not bring any jewelry or valuable items with you to the hospital.

Important

After the surgery, you will need to avoid moving around as much as possible. If the drive home is longer than an hour, arrange to stay overnight somewhere near the hospital.

If you’ve had general or regional anesthesia, you cannot drive for 24 hours after the surgery.

The day of your surgery

When you get up

  • Do not eat or drink anything.
  • Take your usual medications with a sip of water, EXCEPT for the ones indicated by the preadmission nurse.
  • Take a bath or a shower. Do not apply makeup or nail polish, as this will prevent the surgeon from monitoring your skin colour during the procedure.
  • Remove jewelry, contact lenses, hair clips and body piercings.

When you get to the hospital

  • Arrive at the scheduled time in Day Surgery (you must be fasting), accompanied by a family member or friend.
  • Advise the nurse of any change in your health condition.

Next steps

  • The Day Surgery nurse will admit you and answer your questions, then complete the preparations for the surgery and fill out your medical record.
  • An attendant will bring you to the operating block.
  • After the surgery, you will be taken to Day Surgery or to your room, where we will take care of you until you are discharged.

After the surgery

Your discharge

You can leave the hospital as soon as your condition is stable. A nurse will give you the standard recommendations for your health condition and type of surgery.

Make sure you have:

  • your health insurance card and hospital card;
  • the prescriptions and recommendations from your doctor;
  • your personal medications;
  • the details of your next doctor’s appointment;
  • a number to call in case of a medical emergency.

Pain and pain killers

You will be given pain killers after the surgery. If you are in pain, don’t hesitate to tell the nursing staff. Proper pain relief is important to your recovery process. We encourage you to get up quickly to avoid postoperative complications.

Recovery

Your surgeon and the nursing staff will give you information about your recovery. If you have any questions or concerns, do not hesitate to talk to them.

Description

The diabetes clinic gives classes for diabetics and their families on topics to help them better manage the disease and understand its complications. These classes are given by experienced professionals, including nurses and nutritionists.

Topics include:

  • capillary blood glucose testing;
  • diet;
  • carbohydrate counting;
  • taking medication;
  • etc.

For more information about the different classes, call the Centre d’enseignement. Talk to your family doctor, who will have to fill out the registration form.

Your admission to the hospital

Admission

When you arrive, you or a family member must report to the Admission office.
Where: main entrance
Office hours: Monday to Friday, from 8:30 a.m. to 12 p.m., and 1 p.m. to 4 p.m. (except holidays)

Room choice

Our hospital has two types of rooms:

  • Semi-private rooms with two beds
  • Wards with four beds

You are not guaranteed the room you choose at the time of your admission. However, we will try to respect your preference. During your hospital stay, you can always change your mind by filling out the “Modification du choix de chambre” (Room change) form. 

Valuables

The hospital is not responsible for the loss or theft of your personal items, including your hearing aids, dentures and glasses. We recommend you give your valuables to a family member to keep for you. If you’d prefer to keep your items at the hospital, Security can keep them safe until you are discharged.

Your discharge from the hospital

Discharge procedure

Your doctor must sign your hospital discharge form. In general, the document is signed 24 hours before your discharge. You must leave your room by 10 a.m.

Hospital fees

At the end of your hospital stay, go to Accounting to inform the clerk that you are being discharged and to settle any bill related to your stay.

Hospital fees are fully covered for Québec residents with a valid health insurance card, with the exception of certain treatments and medical services not covered by the RAMQ. Your doctor will let you know if this is the case. Speak to your insurer about the terms of the reimbursement.

Description

Sleep apnea is a serious disorder that causes your breathing to stop repeatedly while you sleep. These breathing pauses or “apneas” usually last 10 to 30 seconds and can happen many times throughout the night. 

Snoring by itself doesn’t necessarily mean that you have sleep apnea. It is true that loud snoring is common in people with this disorder, but there’s a big difference between simple snoring and sleep apnea.

With a prescription from a doctor, the sleep laboratory can diagnose sleep apnea, manage users with the disorder, and provide appropriate treatment.

The Emergency department is open 24/7

Do you have an urgent health problem? Our Emergency department can provide you with the care and services you need, at all hours of the day and night.

When should I go to the Emergency department?

Go to Emergency if your health problems require IMMEDIATE TREATMENT.

If there’s no immediate threat to your health, you’re best to call your family doctor, specialist, local CLSC or Info-Santé (811), or go to a walk-in clinic near you.

What do I need to know about the Emergency department?

Our medical teams in the Emergency department triage, assess, stabilize, examine and treat patients.

Triage
When you arrive at the Emergency department, a nurse will assess you to determine the urgency of the situation and the level of care you need. Your condition will then be rated on a priority scale. The nurse may reassess you periodically or at your request if your condition changes.

Team

The team is made up of various professionals trained in emergency medicine, who are constantly honing their skills. With access to leading-edge equipment, they are dedicated to making the user’s stay in Emergency as smooth as possible.

Access to the Emergency department is restricted to the user and their family.

Remember

You must have your health insurance card with you at all times. Otherwise, you will have to pay for the care you receive and submit an application for reimbursement to the Régie de l'assurance maladie du Québec.

Description

There are no dedicated palliative care beds at Hôpital Barrie-Memorial. However, terminally ill patients and their loved ones can count on the services of the medical, nursing and basic care teams, not to mention an occupational therapist, physiotherapist or social worker, if needed. Users who do not want to die at home will be referred for admission to one of the dedicated beds on the territory.
